GENERAL RESPONSIBILITIES:
Provide general oversight of Tobacco Use Cessation Program (TUCP) including coordination of referral and intake process, provision of individual and group counseling and management of documentation and reporting.
SPECIFIC RESPONSIBILITIES:
1. Participation in training to become expert in DHHS Clinical Practice Guidelines regarding tobacco use cessation.
2. Coordination of marketing and dissemination of materials to promote the TUCP targeting low socio-economic status and lesbian, gay, bisexual and transgender populations.
3. Establishment and maintainance of referral network.
4. Coordination of referrals and faciliation of TUCP intake process.
5. Development of individual treatment plans for TUCP participants..
6. Provision of individual and group counseling.
7. Coordination of referrals to TUCP consulting physician for nicotine replacement therapy evaluation.
8. Coordination of referrals to the CT Quitline for additional supportive cessation services.
9. Coordination of data collection and entry into ACCESS database.
10. Coordination of quality assurance activities.
QUALIFICATIONS:
- Bachelors in Social Work, Public Health or other social services related field preferred.
- State certification as Substance Abuse Counselor and/or two years experience in substance abuse counseling preferred.
- Knowledge of the community and its resources.
- Good oral and written communication skills.
- Experience in group facilitation.
